Athletic Handbook
The David's Star Athletic Handbook contains guidelines, policies and general information about the athletic opportunities offered to students.  Athletes and their parents are expected to know and abide by these policies.  Questions about this handbook should be addressed to the athletic director.
Athletic Consent Form
The athletic consent form must be read and signed by each athlete and parent before participating in athletic activities at David's Star.  The bottom portion of the form should be returned to the school office or athletic director.
Coach/Volunteer Form
This form can be used by adults interested in helping our athletic program by volunteering.  There are sections for coaching and other general opportunities, such as scorekeeping, reffing or ticket taking.  Without the help of parent volunteers, David's Star would not be able to run an effective athletic program.  Please consider helping.
DS Coaches Handbook
The DS Coaches Handbook contains policies and procedures to know for becoming a coach at David's Star.
NEW Concussion Policy - Wisconsin Law Act 172 - Statute 118.293
This law requires all youth athletic organizations with participants under the age of 19 to educate coaches, athletes and parents on the risks of concussions and head injuries.  Also, no athlete will be allowed to participate in youth sports until the athlete and parent or guardian has returned a signed agreement sheet indicating they have reviewed the concussion and head injury informational materials. Due to the potential seriousness of any traumatic injury to the brain, the law mandates that any individual with a possible concussion must be removed from competition and not allowed to participate again until cleared by a health care professional with experience in treating head injuries.

At the beginning of each athletic season, all participating athletes and parents must read the Concussion ABC's for Parents/Athletes and sign the Parent & Athlete Concussion Agreement form. No person may participate in a youth athletic activity unless the person returns the agreement signed by the student and by his or her parent or guardian.

Please make sure to submit the Parent & Athlete Concussion Agreement to the school office or directly to the athletic director.   Student athletes will not be allowed to practice or play until this form has been submitted.
